I think you need to abreviate. you don't need this Self-assessment, CV should not be like this.
you don't need ro write this kind of skills like Excel or Word because now everybody has the same skills.
about your grammar is ok.
CV should be abreviated like this:
personal informations
educational backround
professional backround
obtained diplomas.
if you have a good skills that will great if you don't have ,don't write anything like Word Excel, just write computer skills
